Compression Stockings
Treating and preventing DVTs can be done with several methods. One common method is for people to use compression stockings. These stockings function by applying external pressure to the legs that reduces swelling and assists the leg (calf) muscles to pump blood. This added pumping action helps the patient avoid developing DVTs. They may also reduce the pain and swelling in a person who has developed a DVT
 
DVT: Home Care
Wearing compression stockings is useful not only in the hospital setting but also at home, especially if your doctor recommends them. In addition, if you have the opportunity to raise your legs up (a height equal to or above your heart level while reclined), this is a good way to reduce leg edema and aid blood circulation in the legs.
 
Anticoagulants
Anticoagulants, such as heparin, low-molecular-weight heparin, or Coumadin, have been the main treatments for DVTs. This is traditional treatment of treating DVT in india, but this anticoagulant are blood thinner, it prevents further clot formation. It will not reduce thrombus burden which is already formed. So if pt. treated with only anticoagulant it will develop post thrombotic syndrome.
 
Advanced Treatment Of Dvt Are
1) Advanced catheter directed thrombolysis(CDT) treatment:
In catheter directed thrombolysis, we directly inject a clot bursting agent(thrombolytic agent like streptokinase, urokinase, RTPA etc) into the blood clot through a specially designed catheter(small plastic tube) that is placed into pt’s leg vein. We do aspiration of thrombus simultaneously. These thrombolytic agents are given for 12-36 hrs of time slowly & it is monitored in cath-lab periodically.
This thrombolytic medicine will reduce clot burden & clear the passage of vein. It will reduced edema & pain immediately. It will also prevent damage of deep venous valve , so it will prevent post thrombotic syndrome.
CDT is usefull when pt presented in acute stage(i.e. within 14-21 days of DVT formation). If clot age is less, CDT is very effective. Once clot is organize, thrombolytic agents can not lyse thrombus.
 
2) Percutaneous meachanical thrombectomy + CDT:
In this method, along with CDT, MECHANICAL ASPIRATION DIVISE ARE USED TO aspirate the clot. Combining CDT and percutaneous mechanical thrombectomy (PMT) devices has been attempted to try to improve early mechanical thrombus removal and promote lysis of remaining clot.Several different devises are available, but two main devises which are widely used are trellis & angiojet.
 
2) TRELLIS:
 
In trellis, balloons are inflated at proximal & distal end of thrombus, than rotating wire will crush thrombus inbetween this balloon. These thrombus will be aspirated & thrombus burden will be reduced.
